Hospice care provides access to in home medical, social and spiritual resources for patients at the end of life, aimed at making the patient and their family comfortable and confident in the journey ahead.

We’re with you for the entire journey — and we’ll be here to provide grief support before, during, and well after your loved one passes.
What is Hospice Care?
Circle of Care
When working with OC Hospice, you’re never alone. Starting from your first in-home visit, you’ll be assigned a dedicated care team — a physician, nurses, hospice aides, a medical social worker, spiritual counselor of your choosing, and volunteers — to work closely with you and your family on a plan of care.
All of our patients have access to all of their hospice benefits - and every type of care needed throughout this process.
Equipment & Supplies
We have everything you’ll need for the care of your loved one. We will provide all equipment deemed necessary - hospital bed, wheelchair, walker, bedside table, commode, shower chair, cane, Medic-Alert alarm, and others to personal items like lotions, diapers/briefs, gloves, soaps, shampoo, hand sanitizer, bandages, wound supplies, bed pads, wipes, disinfectants, etc.
Pharmacy Services
Our staff will handle any coordination of pharmacy services and medications needed for the patient for comfort and symptom management.
Regular Visits
Our highly skilled nurses visit patients a minimum of twice weekly (exceeding Medicare standards of once per week), and are matched based on geographical location to ensure faster emergency response times, low nurse/patient ratios, and consistent care teams.
Patient and Family Support
We provide individual counseling, caregiver support, and grief support to anyone in the family who needs it. Acting as a caregiver for a loved one reaching the end of life requires strength and resilience, but it can also take a toll on your emotional and physical health. No one can, or should, be strong all the time. Let our team provide a safe space to grieve and the support you need to continue to care for your family.
Emergency Care 24 Hours a Day
For any questions you may have along the way, we’re available for emergency care and guidance 24 hours a day, everyday.

Our specially trained volunteers are an important part of providing compassionate care, as well as companionship, assistance, and support.

Every patient is special to us. That’s why we offer to celebrate the memory of each patient we care for with a special “Tree of Life” dedication after their passing.

Our low staff to patient ratios keep our staff available and prepared to provide the type of individualized care that you and your family need. We’re available 24/7 — even on holidays, weekends, and late nights — to admit, evaluate, and care for our patients. That’s the kind of service and reliability family deserves.
Languages We Speak
We want you to feel heard and comfortable when working with us. Our staff speaks several languages to better serve you, including Spanish, English, Tagalog, Farsi, Korean, French, Romanian, Swahili and Vietnamese. If you have a specific language request, please let us know.
Insurance Accepted
Hospice care costs are generally covered under Medicare, Medi-Cal (CalOptima) and most other insurance providers. If you have questions regarding whether or not your insurance covers hospice benefits please reach out to us anytime.
